Clinical Pharmacy Specialist

This position is located in the Pharmacy Service at the West Cobb Facility Veteran Affairs Medical Center, Austell, GA. The clinical pharmacy specialist is the clinical experts and clinical leader within the Pharmacy Department. In addition to maintaining an ongoing and outstanding knowledge base with regard to current literature and contemporary practice. The Clinical Pharmacy Specialist responsibilities include but are not limited to:
Scanning the internal and external environments to determine opportunities to enhance pharmacy services to support patient education, patient adherence, clinical outcomes. and team member growth and development. Routinely benchmark with national contemporary programs and make recommendations for clinical pharmacy service and program changes. Actively participate in supporting any defined educational programs and activities. Meet with new patients entering our system to reconcile medications. review formulary availability and options for conversion, and educate regarding VA policies. Monitor drug utilization (formulary, non-formulary, restricted) and intervene to ensure formulary adherence and optimal evidence-based clinical outcomes. Participate in the development of quality and performance monitoring programs to support the optimal use of our personnel and medication resources. Actively demonstrate a willingness to generate cost containment ideas and then to implement and enforce departmental and/or institutional programs to support the wise use of available resources. Routine review of medical orders for appropriateness of the drug, dosage, schedule. duration, and directions. Objectively support adherence to the formulary and demonstrate the assertiveness and poise that is needed to handle any untoward situations. Support the review and adjudication of non-formulary medication orders consistent with departmental guidelines. Analyze and interpret the published literature, and appropriately apply one's findings. In addition, demonstrate a willingness to maintain a current understanding in terms of the breadth and depth of the literature in appropriate clinical areas. Routine formulation of clinical medication therapy management decisions based on a review of the patients' computerized medication profile, remote computerized patient record system, patient consultations, and provider consultations. Decision analysis may include obtaining a patient medication history and reviewing the medication profile for prescription duplication, potential drug-drug interactions, drug-nutrient interactions. therapeutic duplication, prescription noncompliance, laboratory reports, and allergies / adverse drug reactions. Routine polypharmacy reviews to identify opportunities to reduce medication and therapeutic duplications and enhance patient medication administration compliance. Routinely demonstrate the willingness to identify and address the routine and the most salient needs of our stakeholders (patients, providers and colleagues). Act as a liaison between service line leadership, pharmacy management, medical staff and healthcare personnel, and assigned pharmacy team members. Routine emphasis should be on clinical, operational and environmental-scanning communication updates. Actively support the development, implementation, management and monitoring of clinical and operational guidelines - national, VISN and local. Actively support through personal and team mentoring the professional growth of assigned personnel. Actively support the service line team-based management of the pharmacy service. Demonstrate a willingness to support the actions and decisions of the management team. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education. (TRANSCRIPTS REQUIRED). Licensure:
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico) or the District of Columbia. (COPY OF LICENSURE REQUIRED) English Language Proficiency:

Experience:
In addition to the basic requirements for employment, candidates must have one (1) year of experience equivalent to the next lower grade level(GS-12 Federal grade level). Assignments:
Individuals in these assignments coordinate a single program area of a complex pharmacy operation, or multiple programs areas within a facility, to develop, organize, manage, and control complex pharmacy programs. Comprehensive knowledge of DEA, FDA, VA, the State (Territory of District of Columbia, if appropriate), TJC and other standards related to the distribution and control of scheduled and non-scheduled drugs to both inpatients and outpatients (including research and investigational drugs). Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):

Expert understanding of regulatory and quality standards for their program area. KSA-3:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
